Bridge Care Connections was established to simplify navigation within complex healthcare systems, connect individuals to medical and mental health resources, and advocate for equitable care. We are committed to building bridges that foster unity and long-term community transformation.

Community Health Advocacy
Launched July 2026
Our comprehensive care management framework is currently operational. Engage in educational seminars via Thinkific or attend local sessions throughout Colorado facilitated by our strategic community partners.

A trusted directory connecting you to providers who offer ethical, culturally responsive care for minorities and underserved communities.
Provider Services Support
SUMMER 2027
Advancing health equity through provider education and leadership training. Services will also be billable through Medicaid, with HCPF approval currently in progress.
Colorado Healthcare Fundraising Ball
Spring 2028
Established charitable event for community members and providers to attend annually. Patients and providers will have the opportunity to speak about the barriers they have faced in healthcare and leadership to advance rapport between the community.

Daijha Reed is a Denver native with 10+ years of healthcare and nonprofit experience. Their work spans direct patient care, corporate nonprofit roles, and shelter-based support, giving them a deep, lived understanding of the disparities Bridge Care Connections is challenging.

Ms. Reed retains her Associates of Science and Nursing and a Bachelor of Arts in Public Health, Minor in Communications from Colorado University of Denver. Additionally she is the worldwide best-selling author of "Scarred Petals: A Bloom Stunted by Storms"
Founder Story

Bridge Care Connections was founded from lived experience navigating diagnostic delays, fragmented care, and barriers that too often impact underserved communities. After years of unresolved health concerns and difficulty accessing coordinated, patient-centered care, I recognized how challenging healthcare systems can be—especially for those already facing disparities.

As a Black woman, public health professional, and community advocate, my experiences helped shape a mission focused on improving healthcare navigation, connecting individuals to culturally responsive resources, and supporting more equitable access to care. Bridge Care Connections was created from the belief that everyone deserves to be heard, informed, and supported in their healthcare journey.
